Instructor for Diabetes Program

Job Summary: Provides individualized diabetes self-management education and/or training (DSME/T)  to individuals and groups according to the National Standards for Diabetes Self-Management Education and Support (AADE , revised 2017).  

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Collects DSME/T program participant assessment data, using the AADE7 framework, in a collaborative and ongoing manner.
  • Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSME/T program participants.  
  • Evaluates effectiveness of educational services provided by measuring attainment of learning objectives
  • Documents assessment data, educational plan, and services provided in each participant’s record
  • Incorporates principles of adult learning including collaboration and skill-based methods in the planning, implementation, and evaluation of DSME/T
  • Assists patients with diabetes in navigating the health care system
  • Identifies when a program participant’s needs are outside the scope of the instructor’s practice and expertise, plus arranges for additional services to meet needs.
  • Conducts follow-up assessment upon completion of DSME/T program services using outcome measures from the AADE7 Continuum of Outcomes Framework.
  • Serves as a consultant to inpatients based on daily triage of census as well as referrals

Minimum Requirements: Current State of Michigan RN, RDN or RPh license.  Three (3) years of applied clinical experience as an RN, RDN, or RPh including strong experience with persons with diabetes. Academic preparation and/or experience in program management of persons with a chronic disease. Certified Diabetes Educator within six months of hire or annually accrues 15 hours of approved continuing education based on DSME Program anniversary date. BLS certification.

Knowledge, Skills & Abilities: Excellent organizational, interpersonal, analytical and communication skills and ability to deal effectively with a variety of people and situations. Maintains flexibility when managing multiple roles and responsibilities and is capable of communicating effectively across multiple disciplines. Demonstrates attentiveness to accuracy and ability to produce educational programs and materials relevant to target audience.  Ability to adjust content and teaching strategies to patient, nurse and other learner needs.  Creates a climate that supports collaborative learning across multiple disciplines and roles. Promotes a safe and healthy work environment and serves as a role model to promote excellence and safety. Proficiency in use of a variety of software and computer programs including search engines, word processing, spreadsheets, electronic medical records, databases, queries and e-mail and calendar programs. Knowledge of audiovisual and demonstration equipment set up preferred. Ability to communicate effectively both orally and in writing and handle complex and/or confidential information in an appropriate manner.

Working Conditions: Some travel may be required.

Physical Requirements: Constantly see/visual acuity, talk/hear.  Frequently stand, sit.  Occasionally lift/carry 1 to 49 lbs.
