Nurse Practitioner

Job Summary:  Provides treatment services for patients admitted to the Wound Center in collaboration with the panel and attending physicians.  Provides diagnosis, treatment and consultation as needed for wound patients.  Assess the physical health status of patients. Assists the physician and treatment team in reviewing the condition of the patient and determining the correct treatment options.  Prescribes medications and write orders for the program’s patients. 

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Assesses patient medical data including health histories, diagnoses, and wound treatment histories.
  • Performs physical examinations and consultations for wound patients.
  • Provides wound care services and recommendations based on examination and information compiled.
  • Performs therapeutic procedures for which the provider has privileges, provides direct treatments as clinically indicated, and monitors results of those treatments.
  • Participates in treatment team planning for patients to develop a plan of care that meets wound healing goals.
  • Provides patient education and answers questions and explains procedures.
  • Provides community education when needed and recognizes the importance of building partnerships with all members of the health care team.

  • Participates in department and system-wide continuous quality improvement activities.
  • Appropriately records findings in the patient medical record. Maintains patient charts, enters diagnosis and billing codes, logs data into patient information systems, and updates file systems for tests and treatments performed.
  • Assists the clinical coordinator in unit operations and allocation and management of resources.
  • Performs other tasks reasonably expected of an Advanced Practice Nurse in a wound center program.
  • Performs marketing activities in conjunction with the Medical Director and Marketing Staff.

Minimum Qualifications:   Graduation from an accredited school of nursing with a Master’s or Doctorate degree in Nursing Practice and current licensure as a nurse practitioner in the State of Michigan. BLS required.

Knowledge, Skills & Ability: Shares accountability for healing outcomes in the Wound Center, which includes clinical practices and patient satisfaction leading to the success of the program. Must be self-confident, a team player and be affable/congenial. Experience with an electronic medical record preferred. Must be able to communicate clearly and effectively with patients, doctors and other staff, have the ability to maintain patient confidentiality and possess the ability to work independently and function as an integral member with other healthcare professionals. Must have knowledge of medical terminology, disease processes and basic medical procedures and skilled in monitoring patient responses and assessing and prioritizing multiple tasks, projects and demands. Two years of clinical experience in a wound care program preferred.

Working Conditions:   Primarily works in clean, well lit, air controlled area, not under any extreme conditions.  Able to work around moderate noise level in an office environment.  Potential exposure to infectious disease.

Physical Requirements:   Constantly see/visual acuity, handle/grasp/feel, talk/hear.  Frequently stand, sit, walk, reach, push/pull, bend/twist, stoop/kneel/crouch, taste/smell, lift/carry 1 to 49 lbs.  Occasionally lift/carry 50+ lbs.
