Nurse NavigatorJob Summary: Works closely with physicians in various specialty areas, coordinating an individualized patient plan of care with the patient, caregiver and their families. Under general administrative direction, provides patient education related to their illness and planned treatment. Provides safe and highly reliable processes in a relationship centered environment. Directs and develops staff; collaborates with all disciplines, individuals and departments. Values and maintains departmental process improvement. Essential Functions:
Minimum Qualifications: Associate degree in Nursing. Licensure state of Michigan. BLS certification. Experience in preparing and scheduling of patients for surgery. Knowledge, Skills & Abilities: Knowledge of supportive care principles and techniques for surgery patients. Ability to perform nursing assessments and patient triage. Ability to educate patients and/or families on what to expect throughout surgical experience. Ability to gather data, compiles information, and prepares reports. Ability to maintain quality, safety and infection control standards. Working Conditions: Potentially exposed to infectious diseases and hazardous material. Physical Requirements: Constantly sit, see/visual acuity, handle/grasp/feel, talk/hear, lift/carry 1 to 25 lbs. Frequently reach, push/pull, lift/carry 25 to 49 lbs. Occasionally lift/carry 50+ lbs. |
