Nurse Navigator

Job Summary: Works closely with physicians in various specialty areas, coordinating an individualized patient plan of care with the patient, caregiver and their families.  Under general administrative direction, provides patient education related to their illness and planned treatment.  Provides safe and highly reliable processes in a relationship centered environment. Directs and develops staff; collaborates with all disciplines, individuals and departments. Values and maintains departmental process improvement.

Essential Functions:

  • Facilitate surgical patients care throughout surgical experience
  • Acts as a central point of contact for all navigated surgical patients; including interaction with surgical, medical, nursing, and ancillary services
  • Works closely with surgical specialties offices to optimize scheduling, triage, preparation and follow up of surgical patients
  • Schedules patients for medical or cardiac clearance and anesthesia preoperative visit
  • Provide direct communication with all surgical patients by phone, e-mail, electronic application or personal interview
  • Works closely with Anesthesia to facilitate medical and cardiac clearances, avoid unnecessary labs and procedures
  • Reviews all patient medication and instructs them on preoperative and postoperative use.
  • Performs nursing assessment and triage of all surgical patients
  • Provides education to patients, families, and significant others; acts as an information resources to students, health care professionals, patients and the public
  • Collects and reports data from patient on postoperative complications to Surgeon and Anesthesia.
  • Perform a postoperative visit to all inpatient surgical patients
  • Uses evidence based enhanced recovery models to optimize patient outcomes prior to surgery
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives and safety, environmental and infection control standards
  • Participates in Anesthesia department meetings and goals, along with collaborative record reviews
  • Designs or participates in the creation of health education materials

Minimum Qualifications: Associate degree in Nursing. Licensure state of Michigan. BLS certification. Experience in preparing and scheduling of patients for surgery.

Knowledge, Skills & Abilities:   Knowledge of supportive care principles and techniques for surgery patients. Ability to perform nursing assessments and patient triage. Ability to educate patients and/or families on what to expect throughout surgical experience. Ability to gather data, compiles information, and prepares reports. Ability to maintain quality, safety and infection control standards.

Working Conditions:  Potentially exposed to infectious diseases and hazardous material.

Physical Requirements:   Constantly sit, see/visual acuity, handle/grasp/feel, talk/hear, lift/carry 1 to 25 lbs.  Frequently reach, push/pull, lift/carry 25 to 49 lbs.   Occasionally lift/carry 50+ lbs.
