Clinical Quality Specialist - Home Care

Job Summary:   Responsible for the dissemination of best practice information regarding the management of a variety of clinical processes including outcome and process measures, and continuous performance improvement for Homecare.

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Sets long and short-term quality goals and objectives for the Homecare and Hospice staff, auditing attainment of goals, performance and quality improvement.
  • Defines quality documentation expectations and formulates policy reflecting expectations.
  • Develops and carries out various  quality and auditing activities
  • Develops action plans for correcting deficiencies and provides feedback on improvements and progress. 
  • Coordinates and aggregates the collection of data.  Distributes and/or stores statistics for internal and external use, and prepares reports as requested. 
  • Assists Homecare and Hospice to achieve and maintain ongoing survey readiness in the area of quality and performance improvement. 
  • Assists with review and follow-up of clinical documentation such as OASIS and Plan of Care orders.
  • Assists with policy and procedure review and development of standards and protocols as needed.
  • Meets regularly with the Director and provides updated information regarding quality activities. 

Minimum Qualifications:  Current RN license in the State of Michigan.  BLS certification and minimum two (2) years recent experience in both Medicare certified home health and hospice services and regulatory and quality compliance.

Knowledge, Skills & Abilities:  BSN preferred.  Knowledge of current regulations and standards including CMS, fiscal intermediary, State of Michigan and other regulatory bodies and accrediting agencies.  Good leadership abilities.  Understanding of anatomy/physiology of disease process and patient assessment protocols.  Knowledge of adult learning principles and philosophies.  Ability to collect and analyze data and implement strategies to show sustained improvement. Effective interpersonal skills with strong oral communication skills.  Ability to work independently and capable of solving problems with theoretical and practical approaches. Ability to maintain patient confidentiality, work in a self-directed and collaborative environment.  Strong clinical, organizational, documentation and teaching skills.  Ability to support homecare and/or hospice philosophies.  Cultural sensitivity to patients from a variety of religious and socio-economic backgrounds.  Ability to assess and prioritize multiple tasks, projects and demands, as well as meet deadlines with a high degree of accuracy.  Creates a caring experience for both internal and external customers. Anticipates and responds to customer needs.  Commitment to continuous improvement for customers including safety, quality of care and excellent service, and gives high priority to customer satisfaction.

Working Conditions:  Primarily works in a clean, well-lit, air controlled area. May also enter unpredictable home environments that vary in cleanliness, temperature, behaviors, smoke, animals, and other potential hazards. May be required to drive a car in all types of weather.

Physical Requirements:  Constantly see/visual acuity, talk/hear.  Frequently stand, sit, walk, reach, push/pull, handle/grasp/feel, bend/twist, stoop/kneel/crouch, taste/smell, lift/carry 1 to 50+ lbs.
