Quality Improvement Coordinator - Quality

Job Summary: Coordinates high quality abstraction,  quality improvement projects, and assists with facilitating teams working on processes pertinent to the organization and regulatory compliance.

 

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Performs accurate and timely data abstraction for elements related to Quality Reporting, Value-Based Purchasing, MHA initiatives,, registries, and other projects. Assures core measure validation is completed prior to quarterly data transmission.
  • Maintains knowledge of measure specifications and data definitions.
  • Reviews of clinical metric performance, assesses for trends and underlying causes for deficiencies. Facilitates the improvement of deficient clinical measures and negative trends involving those necessary to improve processes.
  • Produces internal and external performance reports for analyzing including coordination of the physician scorecards.
  • Provides support and education to clinical and medical staff in making decisions regarding appropriate choices to meet core measure criteria.
  • Reviews patient safety occurrences for trends and opportunities for improvement.
  • Chairs and/or attends committee meetings related to patient safety, quality improvement efforts, and pay for performance activities.
  • Coordinates and facilitates quality projects such as PDCA, FMEAs, Lean and other preventive/corrective action activities.
  • Coordinates key quality awareness initiatives such as National Quality Improvement Week, National Patient Safety Week, etc.
  • Assures the adherence of all standards and policies as set forth by Oaklawn and the accrediting bodies for the purposes of licensure, regulation and accreditation.
  • Functions as a resource for ISO and NIAHO standards for staff.

 

Minimum Qualifications: Associate’s degree in nursing or a related allied health discipline.

 

Knowledge, Skills & Abilities:   Bachelor’s degree preferred. Three (3) years’ of relevant experience performing data abstraction, analysis and process improvement preferred. Knowledge of, or experience in, the delivery of patient care needed to perform the duties of the position. Must be detail oriented with strong organizational and analytic problem solving skills, as well as have exceptional verbal and written communication skills. Basic understanding of statistics needed. Experience in PDCA, Lean, and FMEA tools and processes preferred.  Proficient in the use of Microsoft Office Excel, Word, and PowerPoint.  Ability to perform in a self-directed manner within a team-focused framework.

 

Working Conditions:  Work is generally performed within an office environment with standard office equipment.

 

Physical Requirements: Constantly see/visual acuity. Frequently sit, talk/hear, handle/grasp/feel. Occasionally lift/carry 1 to 50+ lbs.
