Nurse Aide - Casual

Job Summary: Works with professional personnel to provide direct personal care for patients in the home.

 

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Follows the Aide Assignment and provides personal care/ADLS such as: bathing, changing of bed linen, shampooing, shaving, mouth care, etc., as assigned.  Maintains a clean, safe and healthy environment.
  • Assists with ambulation, transfers, and/or normal range of motion and positioning as assigned.
  • Obtains vital signs as assigned.
  • Reports any changes in patient/client condition and needs to the Case Manager and communicates with other members of the care team as needed.
  • Maintains patient confidentiality and demonstrates courteous, polite, respectful behavior.
  • Observes basic infection control techniques at all times and respects patient privacy.
  • Documents any missed visits or reasons why a patient refused care.
  • Documents and completes visits notes on paper or in the computer in a timely manner.
  • Performs all duties as assigned and is willing to perform other duties as needed.

 

Minimum Qualifications:  Ability to read, write and carry out directions as would normally be obtained through completion of a high school education. Must meet all CMS training and competency requirements such as:  75 hours of training and/or competency evaluation based on the Medicare Conditions of Participation.  BCLS certification required.

 

Knowledge, Skills & Abilities: CNA certification preferred. Minimum two years related experience preferred. Must have the ability to follow directions, monitor patient conditions, work independently, work in a self-directed and collaborative environment, meet deadlines with a high degree of accuracy, and assess and prioritize multiple tasks, projects and demands. Must have effective interpersonal skills in all areas of communication with strong oral communication skills.  Cultural sensitivity to work with patients from a variety of cultural, religious and socio-economic backgrounds. Creates a caring experience for both internal and external customers, is courteous and respectful, anticipates and responds to customer needs. Is committed to continuous improvement for customers that includes safety, quality of care and excellent service, and gives high priority to customer satisfaction.

 

Working Conditions:  May work in unpredictable home environments with variations in cleanliness, temperature, behaviors, smoke, animals, and other potential hazards. Required to drive a car in all types of weather.

 

Physical Requirements:  Constantly see/visual acuity, talk/hear.  Frequently stand, sit, walk, reach, push/pull, handle/grasp/feel, bend/twist, stoop/kneel/crouch, taste/smell, lift/carry 1 to 50+ lbs.
