Documentation Specialist

Job Summary:  Reporting to the Director of Health Information Management, the Documentation Specialist is responsible for monitoring documentation that is entered directly into the Electronic Medical Record by providers.  Assists with facilitating improvement in the overall quality, completeness, and accuracy for medical record documentation.

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Reviews documentation from multiple areas of the hospital.
  • Develops and maintain tracking scorecards for quality documentation.
  • Communicates with physicians, offices, and staff to ensure quality and accuracy of the medical record.
  • Reports statistical information from documentation review to the Director of Health Information management as well as the Quality and Compliance departments.
  • Goods interpersonal skills to function within a team-based environment to make effective changes when necessary
  • Goods organization and time management skills.
  • Excellent customer service skills necessary to work effectively with various levels of hospital personnel.
  • Learns and maintain knowledge of the deficiency system and suspension process.
  • Assists physicians, offices, and staff with HIM related questions or duties.
  • Obtain, maintain and reporting of department or organization statistical information.

Minimum Qualifications: 18 years of age

Knowledge, Skills & Abilities: Previous experience with Cerner and Excel preferred. Knowledge of computers and misc. software, electronic medical records, skilled in interpersonal relationships and customer service.  Extensive knowledge of HIPAA, Privacy and other various medical record regulations preferred. 

Working Conditions:  Work is generally preformed in an office environment, with standard office equipment available.  

Physical Requirements: Constantly sit, see/visual acuity, talk/hear.  Frequently handle/grasp/feel.  Occasionally lift/carry 1 to 25 lbs.
