Casual Spiritual Care - Hospice

Job Summary: Provides family spiritual support services according to the spiritual plan of care.

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Provides support services both in the home and community to patients and their families, in individual and group settings. Provides assessment of spiritual care needs and develops appropriate written plans of care.  Serves as a member of the Interdisciplinary Team when appropriate.
  • Completes all required documentation, per policy.
  • Participates in agency and system-wide continuous quality improvement activities to improve performance and patient outcomes.
  • Creates a caring experience for both internal and external customers, is courteous and respectful, anticipates and responds to customer needs, and gives high priority to customer satisfaction.
  • Collaborates with Hospice team to ensure organization-wide compliance with legal, ethical, contractual, and regulatory standards such as the accrediting agency, CMS, OIG, Corporate Compliance, MIOSHA, etc.
  • Assists with orienting members of the health care team as requested and exhibits behaviors that promote a positive learning experience.
  • Completes all required in-service and educational training throughout the year.  Demonstrates personal accountability for own continuous growth and development and seeks out appropriate learning opportunities to enhance competency and improve performance.
  • Handles and documents customer complaints and performs follow-up.

 

Minimum Qualifications: Bachelor’s degree in social work, theology, counseling or related field. 

 

Knowledge, Skills & Abilities:  Hospice or other related experience preferred. Experience in hospice, community support services or related activities.  Skills in spiritual support and counseling.  Demonstrates good communication skills and effective spiritual care interventions. Able to work in a self-directed and collaborative environment.  Must exhibit strong clinical, organizational, documentation and teaching skills. Has critical thinking skills and ability to support the homecare and/or hospice philosophies. Demonstrates cultural sensitivity in order to work with patients from a variety of cultural, religious and socio-economic backgrounds. Has a working knowledge of federal and state regulations for hospice.  Able to assess and prioritize multiple tasks, projects and demands.  Able to meet deadlines with a high degree of accuracy. Must have effective interpersonal skills in all areas of communication with strong oral communication skills.  Must be able to work independently and be capable of solving problems with theoretical and practical approaches using good judgment. Ability to maintain patient confidentiality.

 

Working Conditions:  Primarily works in a clean, well-lit, air controlled area.  May also enter unpredictable home environments that may include wide variations in cleanliness, temperature, behaviors, smoke, animals, and other potential hazards. May be required to drive a car in all types of weather.

 

Physical Requirements:  Constantly see/visual acuity, handle/grasp/feel, talk/hear.  Frequently stand, sit, walk, reach, push/pull, bend/twist, stoop/kneel/crouch, taste/smell, lift/carry 1 to 50+ lbs.
