Full time Nurse Aide

Job Summary: Works with professional personnel to provide direct personal care for patients in the home.

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Provides personal care and assists with ADLS such as toileting, and general assistance as assigned.  Maintains a clean, safe and healthy environment.
  • Assists with ambulation, transfers, and/or normal range of motion and positioning as assigned.
  • Rooms patients and obtains vital signs as assigned.
  • Reports any changes in patient/client condition and needs to the nursing staff or provider and communicates with other members of the care team as needed.
  • Maintains patient confidentiality and demonstrates courteous, polite, respectful behavior.
  • Observes basic infection control techniques at all times and respects patient privacy.
  • Documents and completes visits notes on paper or in the computer in a timely manner.
  • Assists as needed in other areas of the clinic which may include answering phones, scheduling of patients, insurance verification and authorization.

Minimum Qualifications:  Must meet all CMS training and competency requirements such as:  75 hours of training and/or competency evaluation based on the Medicare Conditions of Participation. CNA Certification. BLS certification.

Knowledge, Skills & Abilities: Two years related experience preferred. Must have the ability to follow directions, monitor patient conditions, work independently, work in a self-directed and collaborative environment, meet deadlines with a high degree of accuracy, and assess and prioritize multiple tasks, projects and demands. Must have effective interpersonal skills in all areas of communication with strong oral communication skills.  Cultural sensitivity to work with patients from a variety of cultural, religious and socio-economic backgrounds. Creates a caring experience for both internal and external customers, is courteous and respectful, anticipates and responds to customer needs. Is committed to continuous improvement for customers that includes safety, quality of care and excellent service, and gives high priority to customer satisfaction.

Working Conditions:  Generally clean working environment in a clinic setting.

Physical Requirements:  Constantly see/visual acuity, talk/hear.  Frequently stand, sit, walk, reach, push/pull, handle/grasp/feel, bend/twist, stoop/kneel/crouch, taste/smell, lift/carry 1 to 50+ lbs.
