Occupational Therapist

Job Summary: Responsible for assessing, planning, organizing, administering, documenting and providing direct occupational therapy services in accordance with accepted occupational therapy practice and physician’s overall plan of care.

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Assesses patients for occupational therapy needs.
  • Obtains appropriate physician orders, sets measurable goals, and monitors progress toward those goals.
  • Administers occupational therapy treatments to patients.
  • Assigns follow-up tasks to the COTA and supervises implementation of the treatment plan.
  • Provides ongoing COTA supervision.
  • Discharges patients when goals have been met or continued progress is not occurring.
  • Conducts home safety evaluations and makes recommendations to improve safety and prevent falls.
  • Establishes home exercise programs.
  • Provides education and support to patients and families.
  • Maintains and completes accurate documentation and reports for all assigned patients.
  • Communicates patient treatment plans and progress to the referring physician.
  • Communicates with other members of the health care team to keep them updated.
  • Creates a caring experience for both internal and external customers.

Minimum Qualifications: Licensed Occupational Therapist in the State of Michigan. BLS certification.

Knowledge, Skills & Abilities:  One (1) year experience in a home health or hospice setting preferred. New graduates may be considered. Evident in implementation of occupational therapy skills and treatments as ordered by a physician in the plan of care, use of specialized equipment, and ability to monitor patient response to treatment. Understands anatomy/physiology of disease process and patient assessment protocols.  Is skilled in home safety evaluation and recommendations, upper body strengthening, bed mobility, tub transfer technique, muscle re-education, activities of daily living, fine motor training, neuro-development treatment, cursory sensory evaluation and tactile sensation, use of adaptive equipment, work simplification techniques.  Must have effective interpersonal skills in all areas of communication with strong oral communication skills. Able to work independently and be capable of solving problems with theoretical and practical approaches using good judgment. Able to work in a self-directed and collaborative environment.  Must exhibit strong clinical, organizational, documentation and teaching skills. Demonstrates cultural sensitivity to work with patients from a variety of cultural, religious and socio-economic backgrounds. Has a working knowledge of federal and state regulations for homecare.  Able to assess and prioritize multiple tasks, projects and demands, and meet deadlines with a high degree of accuracy. Creates a caring experience for both internal and external customers, is courteous and respectful, anticipates and responds to customer needs. Committed to continuous improvement for customers which includes safety, quality of care and excellent service, and gives high priority to customer satisfaction.

Working Conditions:  May work in unpredictable home environments that may include wide variations in cleanliness, temperature, behaviors, smoke, animals, and other potential hazards. Required to drive a car in all types of weather.

Physical Requirements:   Frequently stand, sit, walk, reach, push/pull, handle/grasp/feel, bend/twist, stoop/kneel/crouch, taste/smell, lift/carry 1 to 50+ lbs.
