Diabetes Education Coordinator

Job Summary: Provides expertise as an educator, consultant, facilitator, change agent, leader and researcher. Using evidence based nursing practice the diabetes education coordinator coordinates services and education for persons with diabetes and implements the educational components of the Diabetes Self-Management Education Program (DSMEP).

 

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Institutes short and long term planning and programs to address identified needs; coordinating educational activities for diabetes related care for both patients and staff throughout the hospital and it’s satellite operations
  • Provides diabetes self-management education/training (DSME/T) to outpatients and inpatients based on best practices and utilizing acquired nursing expertise and evaluates the effectiveness of educational programming
  • Provides oversight of the DSME Program including planning, implementation and evaluation of DSME
  • Provides direction for the selection, and on-going review, of the curriculum and educational materials to ensure they meet the needs of the population targeted
  • Incorporates principles of adult learning including collaboration and skill based methods in the planning, implementation, and evaluation of DSME/T
  • Spearheads evidence-based process improvements and monitors outcomes
  • Develops/coordinates education plans for nursing staff related to initial and ongoing competency/education to address diabetes specific nursing care
  • Acts as a Liaison between the DSME Program staff and  with the Diabetes Advisory Committee for the DSME Program
  • Assists patients with diabetes in navigating the health care system
  • Serves as a consultant to medical and nursing staff on diabetes self-care management and improved glycemic control
  • Develops or assists in developing databases related to diabetes using the AADE7 framework, and tracks and analyzes data for a wide variety of purposes
  •  Conducts follow-up assessment upon completion of DSME/T program services using outcome measures from the AADE7 Continuum of Outcomes Framework
  •  Supports staff development with educational activities (which may include remediation, certification on blood glucose monitoring, and product roll-outs on diabetes related products.)
  • Assists with marketing activities to plan community outreach on diabetes care and education to reach diverse populations and demographics.
  • Serves as an educational consultant and/or otherwise participates in task forces, committees, and projects relating to diabetes related policy and program development
  • Responds to regulatory mandates for research and management of various diabetes identified needs of Oaklawn to promote quality patient care and safety
  • Serves as a consultant to inpatients based on daily triage of census as well as referrals

 

Minimum Requirements: Current State of Michigan RN license. BSN. Three (3) years of applied clinical experience as an RN including strong experience with persons with diabetes. Academic preparation and/or experience in program management of persons with a chronic disease. Coordinator of DSME Program will meet one of the following requirements: 1. Certified Diabetes Educator or annually accrues 15 hours of approved continuing education (CE) based on DSME Program anniversary date. May consider non-CDE if eligible to obtain within six months of starting position. BLS certification.

 

Knowledge, Skills & Abilities: MSN preferred. Excellent organizational, interpersonal, analytical and communication skills and ability to deal effectively with a variety of people and situations. Maintains flexibility when managing multiple roles and responsibilities and is capable of communicating effectively across multiple disciplines. Demonstrates attentiveness to accuracy and ability to produce educational programs and materials relevant to target audience.  Ability to adjust content and teaching strategies to patient, nurse and other learner needs.  Creates a climate that supports collaborative learning across multiple disciplines and roles. Promotes a safe and healthy work environment and serves as a role model to promote excellence and safety. Proficiency in use of a variety of software and computer programs including search engines, word processing, spreadsheets, electronic medical records, databases, queries and e-mail and calendar programs. Knowledge of audiovisual and demonstration equipment set up preferred. Ability to communicate effectively both orally and in writing and handle complex and/or confidential information in an appropriate manner.

 

Working Conditions:  Some travel may be required.

 

Physical Requirements:   Constantly see/visual acuity, talk/hear.  Frequently stand, sit.  Occasionally lift/carry 1 to 49 lbs.
