Clinical CoordinatorJob Summary: Responsible for ensuring quality patient care and appropriate documentation according to agency policies. Assists the Director in coordinating the clinical operations of Hospice as designated. Essential Functions:
Minimum Qualifications: Current Michigan Registered Nurse license. BLS certification. Knowledge, Skills & Abilities: At least one year of hospice or other related experience preferred. Demonstrates good clinical skills and leadership abilities. Must have effective interpersonal skills in all areas of communication. Will spend up to 75% of work time in conversation with health care team members and customers. Must be able to work independently and be capable of solving problems with theoretical and practical approaches using good judgment. Must be able to work in a self-directed and collaborative environment. Has a working knowledge of federal and state regulations for hospice. Requires on-call responsibilities. Working Conditions: Primarily works in a clean, well lighted, air controlled area. Requires driving a car in all types of weather for staff training of visits of clinical personnel. Physical Requirements: Constantly see/visual acuity, talk/hear. Frequently stand, sit, walk, reach, push/pull, handle/grasp/feel, bend/twist, stoop/kneel/crouch, lift/carry 1 to 50+ lbs. |
