Casual Licensed Practical Nurse - Hospice

Job Summary:  Provides end of life care and services to hospice patients and their family under the direction and supervision of a licensed physician, or registered nurse. Performs various direct patient care duties under the direction of the hospice clinical coordinator and director. Assist in the assessment of patients; plans and implements and documents in nursing care under the direction of the RN.

Essential Functions:

  • Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives.
  • Assists in evaluating the condition of the patient.
  • Reviews patient medical records to assure completeness and accuracy, performs necessary assessment functions (B/P, temp, height, weight, BMI, pulse ox, etc.), updates medication and allergy information, questions patient and updates status in EMR.
  • Assists in the collaborative discussion and care planning of the hospice interdisciplinary team.
  • Teaches and councils patient and family on end of life care.
  • Reports pertinent information to appropriate personnel, and records data accurately and concisely in the patient record.
  • Participates in on-call and weekend rotation and answers incoming calls, assists callers as indicated, identifying and responding appropriately to emergency situations, i.e. symptom management crisis.
  • Documents appropriate information on patient in EMR.
  • Pronounces patients per agency protocol.
  • Under the supervision/direction of the provider, orders prescriptions by calling, faxing or electronically sending prescriptions and refill information to designated pharmacy within the scope of licensure.
  • Maintains the standards of care and implements the policies and procedures.
  • Adheres to regulations and standards of accrediting body. Observes universal precautions and complies with OSHA and MIOSHA standards re: blood borne pathogens and biohazard waste.

Minimum Qualifications:  Graduate of an accredited school of practical nursing.  Current licensure by the State of Michigan Board of Nursing. Three (3) years hospice experience and current BLS certification.

Knowledge, Skills & Abilities:   Demonstrated knowledge of computers, insurance and medical terminology preferred. Experience using McKesson preferred. Knowledge of new trends and techniques in nursing.  Clear oral expression; oral comprehension includes listening to and understanding others; deductive and inductive reasoning skills; ability to write clearly; selective attention – able to concentrate for protracted periods; time sharing – able to shift back and forth successfully between two or more activities; manual dexterity – to manage IV tubing, syringes, and other equipment; information ordering skills – patterns of numbers, pictures, words and mathematical operations including medication calculation. Ability to work with diverse patient and staff populations with sensitivity to cultural and spiritual influences impacting patient care. Efficiently manage time with minimal supervision.
