Job Summary: Responsible for the development, implementation, monitoring, and assessment of the organizational Infection Prevention and Control Plan and Program in accordance with current federal, state, local, guidelines and regulations, and accreditation standards.  Plans, organizes, and oversees daily management of infection prevention and control activities while providing leadership and management of key initiatives and strategies to ensure ongoing compliance and continuous improvement.    

Essential Functions:

• Develops, coordinates, executes and monitors a comprehensive, organization-wide infection prevention/control program and associated policies/procedures for identification, investigation and reduction of infection as well as mitigation of associated risk.
• Ensures appropriate evidenced-based policies and procedures are in place and current to address prevention and control mechanisms in all patient care and service areas, including the prevention of the transmission of infection among patients, visitors, and all hospital and off-campus staff.
• Oversees and analyzes on-going surveillance using appropriate infection prevention/control criteria, documentation and investigation through direct observation, concurrent chart review, interpretation of microbiology results, and collaboration with internal and external stakeholders as indicated to facilitate action to prevent or reduce risk of transmission of infectious agents and incidence of hospital acquired infections. 
• Ensures compliance with national standards and regulating bodies including, but not limited to, DNV, CDC, OSHA, FDA, and HCFA. 
• Compiles, interprets, analyzes and reports surveillance data, core measures and other infection-related data including risk assessment, prevention and control strategies; tracks and trends Infection Control data, maintains infection control statistics and confidential files and prepares and disseminates reports for identified committees.
• Oversees reporting of data to external agencies including, but not limited to, NHSN and reporting of communicable disease to local and state regulating bodies. 
• Accountable for the timely and complete review and investigation of all hospital acquired infections while maintaining the confidentiality and security of all hospital, medical staff and patient related data and information. 
• Leads and participates in identified committees and teams to collaborate with leadership, medical staff, and other members of the healthcare team providing expert consultation and guidance to all areas, both clinical and non-clinical. 
• Leads and participates in task forces and quality improvement activities by assessing, monitoring, and measuring HAI and other associated metrics and evaluating outcomes on a continuous basis to address specific infection reduction strategies and insure rapid improvement for infection clusters.
• In collaboration with quality, HR, education, and nursing departments, plans, organizes, implements and evaluates organization wide educational programs for all personnel, including contract, administrative and ancillary staff, to enhance skills, knowledge and awareness of community and hospital acquired infections, avoidance techniques, and preventative measures. 
• Reviews, evaluates and analyzes employee exposure reports.

Minimum Qualifications:   Bachelor’s degree in nursing, public health, epidemiology bacteriology, medical technology or other clinical area with a strong working knowledge of healthcare processes with knowledge of infection control principles and practices. Five (5) years related work experience in infection control and surveillance. Certification in Infection Control (CIC).   Proficiency in Microsoft Suite programs. 

Preferred Qualifications:  MSN or Master’s degree in associated discipline.  Previous experience in integrated care delivery system (acute/outpatient).  Experience with DNV, TJC, CDC, OSHA, FDA, HCFA/0CMS, or other regulatory/accreditation bodies.  Experience in data base management and external reporting systems. 

Knowledge, Skills & Abilities:   Demonstrated skills in teaching, communication, problem solving and writing technical policies/procedures. Demonstrates exemplary oral and written data analysis, presentation and communication skills to be able to interact effectively and work collaboratively with all levels of the organization and medical staff. Demonstrated assertiveness, conflict resolution, negotiation and collaboration skills and customer focused interpersonal skills with ability to work well with other in time of stress or crisis. Knowledge of performance improvement monitoring and development of performance improvement indicators and processes. 

Working Conditions:  Work is generally performed within an office environment, with standard office equipment available. Requires on-site presence, visibility in all settings, making rounds in the hospital and medical group facilities as need arises. 

Physical Requirements:  Constantly see/visual acuity, talk/hear.  Frequently sit, handle/grasp/feel.  Occasionally lift/carry 1 to 50+ lbs.
